NAAC-Intake Form Instructions
Step 1
Click DOWNLOAD Forms to download the forms you need to start the intake process.
Filling out the Intake forms is your first step in the intake process. These required authorization documents and privacy agreements are needed for you to complete our intake process.
Here are some of the forms we will need you to fill out:
Intake Form
Authorization Form
Counseling Agreement
Legal Disclosure
Privacy Policy
Authorization to Pull Credit
These documents give NAAC the consent to obtain financial information that is needed for us to create an effective and personalized plan of action for your needs.
As a HUD approved Counseling agency. We provide consultations and referrals, but we cannot give legal advice.
We may also need some additional documents depending on the program you need assistance in. Some of these additional documents may include:
copy of last 2 years W2s
copy of last 2 months paycheck stubs
current utility bill
Driver’s license
etc.

Step 2
Once you have completed the forms and obtained the necessary additional documents. Click UPLOAD Forms to upload your completed forms and additional documents.
Once you submit ALL your completed forms and documents, one of our housing counselors will contact you to schedule a counseling appointment.
